Immunization Reminder - Students Entering 8th & 12th Grade (2025-2026 SY)

Important Immunization Reminder for Upcoming 8th & 12th Grade Students

Dear Parents and Guardians,

As required by the State of Missouri, all students entering 8th and 12th grade must be up to date
on their immunizations or have a valid exemption form submitted prior to the first day of school.

  • Students entering 8th grade are required to have the Tetanus (Tdap) and Meningococcal (MCV4) vaccines.
  • Students entering 12th grade are required to receive a second dose of the Meningococcal (MCV4) vaccine. Meningitis B vaccine will also be available.

To assist families in meeting these requirements, Katy Trail School-Based Health will be at MCR-II on May 9th to provide immunizations to eligible students.

The necessary consent forms will be sent home with students. If you would like your child to
receive their vaccines at school, please complete and return the consent forms to the school nurse
by May 2nd.

If you have any questions or need additional information, please contact your child’s school
nurse.

Thank you for helping keep our students healthy and ready for the upcoming school year!

Sincerely,

Jessica Johnson, RN- High School Nurse (573) 378-4697
